As the summer season approaches, many individuals begin contemplating travel plans, including potential international excursions. If you happen to have a child who does not have a passport and one parent is unavailable due to factors such as residing out of state or being deployed in the military, certain procedures must be followed.

Specifically, you'll need to ensure the non-applying parent completes Form DS-3053, a crucial document in the passport application process. This form requires notarization, and it's imperative to include copies of the valid ID (Driver License, State ID or Parent's passport) presented to the notary, both front and back, alongside the form. Failure to provide this completed form could result in the passport office being unable to proceed with the application. Additionally, you will need to include the child's birth certificate and two passport-sized photographs.

In instances where only one parent is applying for the child's passport, alternative documentation is necessary to affirm sole parental authority. Acceptable documents include a birth certificate indicating sole parental status, a death certificate of the other parent, or a court order establishing sole physical and legal custody.

It's worth noting that Form DS-3053 can be conveniently filled out online and subsequently printed. However, it's essential to refrain from signing the form until after it has been notarized. Below is the top portion of Form DS-3053.
